Step 1: To access the SD Memory Card License Agreement (CLA), please email SD-3CAdmin@millerkaplan.com with the following:

• Company Name
• Contact Person
• Email Address

Step 2: Please download NDA Form and return two copies of the NDA signed in order to obtain the CLA. 

Step 3: After investigating all the terms and Conditions, please return two copies of the CLA signed with check marks for Selection of Applicable Subpart(s).

Step 4: Please download the Royalty Report Form for reporting.

The Royalty Report Form can only be accessed once the CLA has been executed and a Password has been issued.
